 Hi Austin,

       Majority of the 19.5mm is the acrylic glass protrusion. I believe the real metal case thickness is around 14mm (just an approx). The case is also contoured (cushion shaped) therefore it will not wear as big as say an IWC BP.

       I have a 6 3/4" wrist and it looks fine on my wrist.

 
Here's a wristshot of the slightly larger PAM 232 47mm (im not trying to compare the PAM and the Dievas, just showing you how it looks like on my wrist).
